I've been using Fettle on and off now for 2 years. Thankfully Fettle paired me up with an excellent therapist based on the issues I said I would like to discuss and I've come a long way since then. Match what you're feeling to an approach

You don't need a diagnosis or the right clinical words to use this list. Find the description that sounds most like you, then follow the link. If more than one fits, start with whichever is causing the most disruption right now.

How to know if you need therapy

You don't have to be in crisis. Therapy is a reasonable next step if any of these are true:

  • It has gone on for weeks, not days.
  • It is affecting your sleep, work, relationships, or how you look after yourself.
  • The ways you usually cope aren't working, or are making things worse.
  • People close to you have noticed a change.
  • You keep thinking about getting help.

If you are in immediate danger or thinking about harming yourself, contact emergency services on 112 or 999 now.

Anxiety, racing thoughts, worry loops

CBT is the most widely used approach for worry that keeps looping. It works on the thoughts and habits that keep anxiety going.

Trauma, PTSD, or feeling stuck on one specific event

Trauma-focused therapy helps the memory settle so it stops intruding on the present. EMDR and CPT are two structured options your therapist may use.

Relationship conflict, communication breakdown

When the problem lives between two people, working on it together usually gets further than working on it alone.

Intense emotions, self-harm urges, relationships that swing between extremes

DBT teaches practical skills for riding out big feelings without acting on them, and for keeping relationships steadier.

Feeling stuck, avoiding what matters, out of step with your own values

ACT helps you make room for difficult feelings and move towards what matters to you anyway, instead of waiting for them to pass.

Grief, bereavement, loss

Grief has its own pace. Grief therapy gives it somewhere to go, whether the loss was recent or years ago.

Substance use or a compulsive pattern you can't stop

Addiction support looks at the pattern, what drives it, and realistic steps to change it, without judgement.

Persistent anger or irritability

Anger management works on what sits underneath the anger and on catching it earlier, before it costs you something.

Chronic pain with an emotional or psychological side

Pain, mood, sleep and stress feed each other. Chronic pain therapy targets that loop so pain takes up less of your life.

Worries about a teenager

Teenagers often need a therapist who works with their age group, and a setting where they will actually talk.

Not sure, just low mood or stress that won't lift

Start with individual therapy. Your therapist will help you work out what is going on and will say if a more specific approach would suit you better.

Compare the options side by side

What each approach is best for and what actually happens in a session. Read the linked page for the full picture.

Approach Best for What a session actually looks like Link
CBT Anxiety, worry loops, panic, unhelpful thinking habits Structured and practical. You look at specific situations, spot the thought patterns behind them, and practise different responses. Often includes short exercises between sessions. CBT
Trauma therapy (EMDR, CPT) PTSD, a specific event you can't move past, flashbacks, hypervigilance Paced carefully. Early sessions build safety and coping skills before the difficult material is worked through. EMDR uses guided eye movements; CPT uses structured writing and discussion. Trauma therapy
Couples counselling Conflict, communication breakdown, trust, drifting apart Both partners attend together. The therapist helps each of you say what you mean, hear the other, and work through the arguments that keep repeating. Couples counselling
DBT Intense emotions, self-harm urges, unstable relationships Skills-focused. Sessions teach concrete tools for managing distress, emotions and relationships, then review how they went in real life. DBT
ACT Feeling stuck, avoidance, life not matching your values Reflective and practical. You explore what matters to you, notice where avoidance is getting in the way, and practise acceptance and mindfulness skills. ACT
Grief therapy Bereavement, loss, grief that isn't easing Gentle and led by you. Space to talk about who or what you lost and what life looks like now, without being rushed towards moving on. Grief and loss therapy
Addiction support Alcohol, drugs, gambling, compulsive behaviours Honest and non-judgemental. Sessions look at the pattern, what drives it, and realistic steps to change it. Setbacks are treated as information, not failure. Addiction therapy
Anger management Persistent anger, irritability, outbursts you regret Practical. You unpick what sits underneath the anger, learn to notice the early warning signs, and practise ways of responding before it boils over. Anger management
Chronic pain therapy Long-term pain that affects mood, sleep or daily life Collaborative. Sessions focus on how pain, mood, sleep and activity affect each other, and on strategies that reduce the grip pain has on your life. Chronic pain therapy
Youth therapy Teenagers dealing with anxiety, low mood, school or family stress Age-appropriate and confidential. A therapist experienced with teenagers helps them put words to what is going on, with parents involved as agreed. Youth therapy
Individual therapy Low mood, stress, or something that feels off but doesn't have a name yet Open-ended talking therapy. You bring whatever is on your mind and your therapist helps you make sense of it. A good first step if nothing above fits neatly. Book a session

All sessions are online with IACP-accredited therapists. No GP referral is needed for any of them.

Still not sure?

That's normal. Plenty of people read a list like this and can't place themselves, or place themselves in three rows at once.

You have two easy options. Book a free concierge call and talk it through with our team, who will point you to the right approach and therapist. Or book an individual session and let your therapist help you work it out in the room.

How it works

Frequently Asked Questions

Common questions from people deciding where to start

That is one of the most common reasons people start therapy, and you don't need a clearer answer than that to begin. Start with individual therapy. Your therapist's job in the first session or two is to help you put words to what is going on, and if a more specific approach would help, they will say so. If you would rather talk it through before booking, Fettle's free concierge call is there for exactly that.

Yes. Nothing you choose now is fixed. Many therapists draw on more than one approach and will adjust as they get to know you. If the fit feels wrong, tell your therapist, or contact Fettle and we will help you move to a different approach or a different therapist.

No. You don't need a diagnosis, a GP referral or a formal label to book therapy with Fettle. Most people start without one. If an assessment would be useful, your therapist can suggest it, but it is never a condition of getting support.

That's normal, because these problems overlap. Anxiety and low mood usually travel together, and trauma can show up as anger or relationship trouble. Pick the one causing the most disruption right now and start there. A good therapist will work across whatever else comes up.

Appointments are usually available within 24 hours. All sessions are online, with IACP-accredited therapists, so you can start from home without a waiting list or a referral.

Yes. Research consistently shows that online therapy is as effective as in-person therapy for most concerns. Many people prefer it because there is no travel and they can talk from their own space. Fettle's video platform is secure and sessions are confidential.

What is therapy and how can it help?

Therapy, also known as psychotherapy or counselling, is a collaborative treatment based on the relationship between an individual and a trained mental health professional.

Therapy provides a supportive environment that allows you to talk openly with someone who's objective, neutral, and non-judgmental.

Working with a therapist can help you develop insight into your patterns of thinking and behaviour, build coping skills, work through difficult emotions, and make positive changes in your life.

The Therapy Process: What to Expect

In your first session, your therapist will get to know you and understand what brings you to therapy. This is a collaborative process where you set the pace.

Sessions typically last 50 minutes and take place weekly, though this can be adjusted based on your needs and preferences.

Your therapist will work with you to identify goals and develop strategies tailored to your specific situation. Over time, you'll develop new insights and skills that you can apply in your daily life.

Remember: therapy is a journey, and meaningful change takes time. Be patient with yourself.

What can therapy help with?

Therapy can help with a wide range of challenges, both clinical and everyday concerns.

Common reasons people seek therapy include:

  • Anxiety and panic attacks
  • Depression and low mood
  • Relationship difficulties
  • Grief and loss
  • Stress and burnout
  • Trauma and PTSD
  • Self-esteem issues
  • Life transitions
  • Personal growth and self-discovery
